When emailing regarding an appointment, please indicate several
dates and time frames you prefer so we can attempt to match
those with our schedule. Please also include a number where
we can reach you to confirm a final appointment time.

| Information and Questions |
Many of our cosmetic patients prefer
to get information about cosmetic surgery and ask questions
prior to scheduling their complimentary consultation. We are
more than happy to answer your questions and provide you with
information.
In addition, you can email us at info@duffyplasticsurgery.com.
Of course, Dr. Duffy will need to examine you before he can
provide you with details on what to expect from your individual
surgery as each patient has different needs.

| Patient Information Forms |
As with all medical offices, when you
first come to our practice there are a number of forms which
have to be completed. As all of us have been patients ourselves,
we know what a headache forms can be. To help our patients,
we now offer these forms here on our website. The forms are
in a fillable Adobe Acrobat pdf format. You can:

- Complete the forms online and email them to us by clicking
the email button on the upper right corner of the form;
or
- Print the forms, fill them out by hand, and fax to our
office at 972-566-3999; or
- Print the forms, fill them out by hand, and bring them
to your appointment.

Please be sure to still bring your insurance
cards with you so we can make copies of them. |
