The patient information forms can be printed for bringing to our office or faxing. They can also be sent by email. However, because various computer systems may cause an email attachment to be "lost" at one end or the other of the transfer, we strongly recommend you print a copy to bring with you the day of your appointment even if you are emailing the information to us. This will avoid you having to complete the information again if the email does not make it through.
